Will AI Replace Medical Billing Companies? An Owner's Honest Read

An owner-level read of how AI pressures billing companies in 2026 and five moves to make this quarter.

Search "will AI replace medical billing" and Google answers a different question. The "People also ask" box is about jobs: Is medical billing and coding going away? Coders and billers worrying about their careers fill r/CodingandBilling (thread from 2025).

If you own a billing company, your question is narrower and harder. Will AI let someone take your clients, undercut your fee, or buy your firm out from under you? And what should you do about it this quarter?

Here is what the public record shows as of October 2026, and where independent firms still have the stronger hand.

What is actually happening in 2026

Four things are moving at once.

Venture money is going to AI revenue-cycle companies, and most of them sell to practices, not to you. Candid Health raised a $120M Series D in July 2026 (HLTH, Jul 2026). It automates claim review and correction before submission, and it sells to healthcare providers.

Some AI companies are buying billing firms. Procode AI raised $4M and acquired The Auctus Group, a billing firm serving plastic surgeons and dermatology practices with 300+ providers, on March 2, 2026 (HIT Consultant, Mar 2026). Auctus's founder put it bluntly in the same article: "Most RCM AI products are vaporware."

At least one AI-first billing company is attacking on price. Quill Bills, a Y Combinator company that sells billing to private-practice doctors and MSOs (management services organizations), advertises a flat 1.8% fee for its next 10 clinics, against what it calls agency fees of "2% to 10%" of revenue (Y Combinator profile, accessed Oct 2026, vendor figure).

Payers are ahead of providers on AI. The CAQH Index reports that more than half of health plans, but only 25% of provider organizations, use AI tools in administrative work (CAQH Index via Dataspring, Feb 2026). HBMA (the Healthcare Business Management Association, the trade group for billing companies) has run a session on how carriers use AI in creating denials (Fall 2025), and its November 12, 2026 webinar is on downcoding as a payer trend.

Three ways AI pressures a billing company

1. Price

Most billing companies charge a share of what they collect. One pricing guide puts the overall range at 4-10% of net collections, with 5-8% typical for small and mid-sized practices (Neolytix, Sep 2026, vendor figure). A 1.8% offer does not have to win the client to hurt you. It only has to show up in your client's inbox before your renewal.

2. Ownership

If AI-backed buyers can buy a firm and run it with fewer people per account, the firm's value to them is its client list. The Procode deal is the US example. A non-US example: WELL Health's WELLSTAR bought two Canadian billing firms with roughly CAD 5M revenue and EBITDA margins approaching 20%, which Levin Associates counted as the 10th revenue-cycle deal of 2026 across the eHealth market (Levin Associates, Mar 2026).

3. The other side of the claim

When payers use AI to review, deny or downcode, the volume and pattern of denials change. Your billers feel that first.

Where independent firms still win

None of this makes the independent firm obsolete. It changes what you are paid for.

  • You know the payers. Phone and portal work still exists because some payers lack electronic support for certain checks, some smaller third-party administrators skip real-time checks, some payer contracts "discourage or prohibit portal scraping," and some payers have cut portal functions (SuperDial, Jul 2026, vendor source). That mess is local knowledge, and it is yours.
  • You are accountable. Regulators keep a person in the loop on the payer side too. Medicare's WISeR prior-authorization pilot uses AI "along with human clinical review," and non-payment recommendations come from "appropriately licensed clinicians" (HCC Buddy, Jul 2026). Practices want a named person who answers for their money. That is you.
  • You already hold the data. Years of denials, remittances and successful appeals are exactly what an AI drafter needs. A practice switching to a new vendor starts from zero.

The risk is not that AI does your job. The risk is that a competitor uses it to carry twice the accounts per biller and prices accordingly.

What to do this quarter

  1. Measure accounts per biller and time per denial for two weeks. Without a baseline, you cannot tell a client, a buyer or yourself what AI changed. See Accounts per biller: the number to measure.
  2. Write a one-page AI tool policy. Your staff are likely already pasting things into chatbots. Decide which tools are allowed with patient data. See the 2026 BAA checklist.
  3. Pick one queue and start there. Denials are the best first target: you already have the data, and the work is sorting and drafting, which a biller can check. See Denial triage and appeal drafts.
  4. Decide build, buy or partner for payer calls. Several vendors sell AI phone agents for claim status. You probably should not build one. See Build, buy or partner.
  5. Prepare your answer to the 1.8% pitch. Have your own numbers ready: collection rate, days to payment, denial overturns. Price is only one line in that conversation.

What HBMA members are already hearing

HBMA's own programming tells you where owner attention is. Its AI recordings include "Build or Buy: A Practical AI & RPA Playbook for the Modern RCM Owner/Operator" (April 2026) and "The Road to ROI: Where is AI Delivering Revenue Cycle Results?" (March 2026) (HBMA Store, accessed Oct 2026). On November 3, 2026, it hosts a webinar titled "The AI Operating System for RCM Companies" (HBMA calendar).

Owners are being sold AI strategy from every direction. The useful filter is simple: does this make my billers carry more accounts, with a biller still approving every outbound item, on tools that sign a business associate agreement?

The honest bottom line

AI will not replace billing companies as a category. It will make some of them much cheaper to run, and those firms will set the price. Whether yours is one of them depends on decisions you can make in the next 90 days with data you already own.
